Hospice Care Coordinator

About the Role:
VITAS Healthcare seeks a compassionate and skilled Registered Nurse to join our interdisciplinary team. As a member of this team, you will work closely with patients, families, and healthcare professionals to provide holistic care and support throughout the end-of-life journey.
Key Responsibilities:
• Coordinate patient care through effective communication with the Interdisciplinary Group (IDG) and other stakeholders
• Develop and implement individualized plans of care that address the physical, medical, psychosocial, emotional, and spiritual needs of patients and their families
• Conduct comprehensive assessments to identify areas for improvement and ensure continuous quality of care
• Provide education and training to patients, families, and caregivers on palliative care and symptom management
• Collaborate with the IDG to monitor patient outcomes and make adjustments to the plan of care as needed
Requirements:
• Current licensure to practice nursing in the state where the VITAS program is located
• A minimum of two years of experience in hospice, home health, or community health within the last five years
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment and prioritize multiple tasks
