Key Responsibilities:
  • Administer skilled nursing care to patients in their place of residence, promoting comfort, dignity, and quality of life.
  • Conduct patient admissions, ensuring thorough assessments and tailored care plans.
  • Routine patient visits will involve monitoring progress, addressing concerns, and adjusting care as needed.
  • Collaborate with the interdisciplinary team to coordinate care, address family needs, and ensure seamless transitions.
  • Effective communication with physicians, including attending physicians, hospice physicians, and other involved healthcare providers, is essential.
  • Demonstrate accountability, flexibility, respect, and integrity in all interactions.
Required Skills and Qualifications:
  • Current Registered Nurse license in the state of Texas.
  • Prior experience as a RN Case Manager in the hospice setting is preferred but not required.
  • A valid Texas Driver's license and dependable transportation are necessary for home visits.
