Hospice Triage Nurse

At Good Shepherd Hospice, we're seeking a compassionate and skilled Hospice Triage Nurse to join our team. This is a part-time opportunity that requires the ability to work independently and make critical decisions in a fast-paced environment.
Responsibilities:
* Respond promptly to phone calls from patients and families, providing support and guidance as needed.
* Conduct patient visits to assess their needs and develop care plans that maximize their comfort and health.
* Collaborate with the hospice team to ensure seamless communication and coordination of care.
* Train patients and caregivers on various aspects of hospice care, including symptom management and end-of-life care.
Qualifications:
* A minimum of one year of experience in hospice care is required, with a focus on triage and patient assessment.
* Possess a valid multi-state license and be able to pass background checks.
* Strong organizational and self-management skills are essential for this role, as well as good judgment and critical thinking abilities.
* Commitment to providing excellent customer service to patients and families is paramount.
Estimated Salary: $70,000 - $90,000 per annum, depending on experience and location.
