- Conduct daily assessments, prioritize care, and revise patient care plans as needed.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in performance improvement activities to enhance patient outcomes and achieve performance metrics.
- Current license to practice as an LPN in the Commonwealth of Kentucky.
- CPR/BLS certification (American Heart Association).
- Completion of an LPN program at an accredited school of nursing.
- Prior acute care or rehab experience preferred.
