Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team as a Patient Care Manager Assistant. As a key member of our home health care team, you will play a vital role in ensuring the continuity of high-quality care to our patients.

Responsibilities

Assist the Patient Care Manager in coordinating a team of staff to provide quality care to patients in their homes.
Ensure that the team is committed to providing care in an efficient manner, in accordance with physician's orders and State/Medicare guidelines.
Prepare patient lists and necessary documents for weekly case conferences.
Provide direct patient care, as necessary, in accordance with scope of practice, plan of care, and physician orders.
Assist the Patient Care Manager in clinician documentation review/audits to ensure complete, accurate, and timely submission of all documentation by field staff.

Requirements

Current LPN licensure in the state of practice.
Current CPR certification.
Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
