Home Health LPN Job Overview

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in the comfort of their own homes. As a LPN Home Health Specialist, you will have the opportunity to make a meaningful impact on the lives of patients and their families.

Responsibilities:

  • Verify plan of care with the RN prior to each visit
  • Collaborate with physician and RN frequently to ensure patient needs are being met
  • Teach patients and caregivers self-care techniques
  • Check vital signs and review all medications
  • Perform specialized procedures ordered by physician
  • Prepare equipment and materials for treatments using aseptic technique
  • Complete documentation timely and accurately

Qualifications:

  • One year of clinical LPN experience
  • Current LPN license, specific to the state(s) you are assigned to work
  • Current CPR certification
  • Valid driver's license, reliable transportation and liability insurance
