Job Overview

The Care Partner serves as a vital support role under the supervision of the Registered Nurse. The primary objective of the Care Partner is to enhance the patient experience by collaborating with the nursing staff, physicians, and the broader healthcare team to deliver exceptional care. Responsibilities include assisting patients with daily living activities and comfort measures while ensuring a safe and culturally sensitive environment.

The Care Partner may need to possess specific advanced technical skills, such as phlebotomy, basic treatments, dressing changes, oral suctioning, and the removal of Foley catheters. The scope of duties may vary based on unit requirements. Additional responsibilities include clerical tasks like data retrieval, maintaining inventory, and transporting supplies or medications. The role demands the ability to thrive in a fast-paced setting, requiring effective communication and multi-tasking skills while maintaining a compassionate and respectful demeanor.

Key Responsibilities and Essential Functions

Delivering Excellence in Patient Care: Collaborate with the healthcare team, patients, and families to ensure a patient-centered approach. Address patient needs and adhere to the care plan as directed by licensed professionals. Engage patients and families in their treatment plans while consistently implementing safety measures to achieve desired outcomes.

Upholding Patient Rights: Protect the dignity and confidentiality of patients in accordance with HIPAA regulations. Participate in regular patient check-ins and respond promptly to calls and inquiries, escalating concerns to the charge nurse when necessary. Offer proactive assistance and comfort to patients and their families, presenting oneself in a professional manner.

Providing Patient Care: Assist with various patient care needs, including hygiene, nutrition, and comfort measures, as well as advanced skills based on unit requirements. Support the RN and healthcare team in care delivery and monitor key patient safety information.

Team Collaboration: Contribute to the healthcare team's initiatives and participate in performance improvement activities. Support nursing practices through shared leadership and ethical accountability. Assist in the transportation of supplies and equipment as needed.

Effective Communication: Utilize strong verbal and written communication skills with patients, staff, and visitors. Document patient care activities accurately in the Electronic Medical Record (EMR) and communicate any concerns or emergencies to the RN or Charge Nurse promptly.

Safety and Infection Control: Ensure a safe environment for patient care by adhering to proper hygiene practices and infection control protocols. Maintain cleanliness in patient areas and utilize safe handling techniques during patient interactions.

Required Qualifications

  • Rising Senior in Good Standing or GED Preferred

Licenses and Certifications

All certifications must be obtained upon hire unless specified otherwise.
  • Basic Life Support (BLS) certification from the American Heart Association

Preferred Qualifications

  • Completion of a State-approved program for Certified Nursing Assistants or Patient Care Technician upon hire

Required Skills

Ability to follow detailed instructions, strong interpersonal skills, clear communication abilities, and the capacity to work both independently and collaboratively within a team. The minimum age for employment is 16.
