Job Summary

We are seeking a highly skilled and experienced Case Manager to join our team at Houston Methodist Specialty Physician Group. As a Case Manager, you will play a critical role in ensuring that our patients receive high-quality, patient-centered care.

Key Responsibilities
  • Develop and implement comprehensive care plans for patients with complex medical needs
  • Collaborate with healthcare teams to ensure seamless transitions of care and optimal patient outcomes
  • Conduct thorough assessments of patients' medical, social, and financial needs
  • Identify and address potential barriers to care, including insurance and financial issues
  • Develop and maintain relationships with community resources and healthcare providers to ensure access to necessary services
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements
  • Bachelor's degree in nursing or related field
  • Minimum of 5 years of experience in case management or a related field
  • Strong knowledge of healthcare regulations, including Medicare and Medicaid
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Proficiency in Microsoft Office Suite
