Job Summary:

The Manager of Care Management is responsible for assisting with all the daily operations of the Care Management Department. The Manager assists the Department Director with employee supervision, management, and training; data research and analysis; development and implementation of quality initiatives designed to support defined objectives. Guides the team in resource management. Maintains the standard for a collaborative process of assessment, planning, facilitation and advocacy for patients in our care. Individualizes services to meet patient's health care needs by utilizing available resources to promote quality outcomes. The Manager will act in the role of Department Director when the Director is out of office.

Responsibilities:
  • Supervise and manage the daily operations of the Care Management Department.
  • Assist the Department Director with employee supervision, management, and training.
  • Conduct data research and analysis to inform quality initiatives.
  • Develop and implement quality initiatives to support defined objectives.
  • Guide the team in resource management and maintain the standard for a collaborative process of assessment, planning, facilitation and advocacy for patients in our care.
  • Individualize services to meet patient's health care needs by utilizing available resources to promote quality outcomes.
Requirements:
  • Minimum of a Bachelor's Degree in Nursing.
  • The RN License is in good standing in the state in which the job is performed.
  • Minimum of 3 years experience in care management, utilization review, managed care, or clinical operations experience. Experience must be in an acute care hospital and/or managed care setting.
  • Must be familiar with federal, state and accreditation standards as they relate to care management and utilization review.
Preferred Qualifications:
  • Case Management Certification preferred.
  • Supervisory experience preferred.
