LPN - Clinic I (Meds) Healthcare Professional

Key Responsibilities:
* Review and prepare medical records for assigned patients, initiate follow-up for medical records as requested, and review patients' needs.
* Identify and prepare orders for patients' needs for authorization by the Practitioner and may accept verbal orders per policy.
* Observe, collect, and document new information on patients, inform RN or Practitioners regarding incomplete data, new observation, or abnormal/unusual data.
* Implement basic nursing procedures per plan of care, administer treatments, immunizations, and medications as prescribed by the Practitioner.
* Prepare medications for the Practitioner to use, perform point of care testing as authorized, and provide laboratory, x-ray, and other reports to the Practitioner for review.
* Notify patients of normal results as directed by the Practitioner, consider special needs of different age groups when providing care, and set priorities of care based on acuity of patients.
* Document implementation of orders/interventions, record findings, instructions, and patient responses on appropriate forms or online records using department and/or JCAHO standards.
* Observe patients/family responses to the plan of care, report unexpected responses to RN or Practitioners, and collaborate with RN/Practitioner to revise care plan when needed.
Requirements:
* Twelve (12) months (2080 hours) LPN experience.
* Hawaii State Board of Nursing approved curriculum for LPN licensure.
* Licensed Practical Nurse (Hawaii) required at hire.
* Basic Life Support from American Heart Association.
* Demonstrated knowledge of and skill in conflict resolution, customer service, interpersonal relations, oral communication, problem solving, teamwork, and written communication.
