Care Management Assistant Job Summary
The Care Management Assistant plays a vital role in ensuring patients receive quality, cost-effective care. This includes screening, assessing, and monitoring patient needs, determining treatment eligibility, and making arrangements for additional services to achieve positive outcomes and close gaps in care.
Key Responsibilities:
- Help PCPs and/or clinic staff manage high-risk patients by providing timely information on preventive screenings and progress to reduce gaps in care.
- Assist patients with referrals for Behavioral Health, Healthcare Management, Tobacco Cessation, Substance Abuse counseling, and track compliance and follow-through.
- Participate in team meetings and quality improvement initiatives, assisting with patient data collection, health outcomes reporting, clinical audits, and programmatic evaluation.
- Knowledgeable with management and reporting tools, including educating PCPs and clinic staff on best use.
- Understand and use utilization management reports to track gaps in care.
- Knowledgeable with Medicare's chronic care guidelines and Medicare's Innovation Programs and Delivery Guidelines, also understanding billing associated with these guidelines.
- Perform miscellaneous responsibilities and duties as assigned.
Requirements:
- 2-5 years of experience in a clinical setting with a Medical Assistant Certificate.
- Ability to manage multiple tasks simultaneously, maintain a positive attitude, and work overtime or after hours as needed.
- Self-disciplined, energetic, passionate, innovative, highly organized, and well-developed oral and written communication skills.
- Sound judgment, decision-making, and problem-solving skills, with the ability to maintain confidentiality.
Preferred Qualifications:
- Effective communication and telephone skills.
- Proficient computer skills, including Microsoft Office (specifically Word and Excel, Coreo, Webchart, etc.).
