Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Evergreen Life Services Texas. As an LPN, you will play a vital role in providing high-quality patient care and support to individuals with intellectual and developmental disabilities.

Key Responsibilities
  • Provide Direct Care: Administer medications, perform wound care, and provide other skilled nursing treatments as needed.
  • Assess and Document: Conduct thorough assessments of patients' health needs and document findings in a timely and accurate manner.
  • Collaborate with Team Members: Work closely with other healthcare professionals, including RNs, to ensure comprehensive patient care.
  • Maintain a Safe Environment: Ensure a safe and healthy environment for patients, staff, and visitors by following infection control procedures and reporting any concerns or incidents.
  • Communicate Effectively: Communicate clearly and effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in Quality Improvement: Contribute to quality improvement initiatives and participate in ongoing education and training to enhance patient care and services.
Requirements
  • Education and Experience: Graduate of an approved Licensed Practical Nursing program and a minimum of one year of clinically relevant experience.
  • Skills and Knowledge: Strong knowledge of medical nursing procedures, first aid, and basic principles of nursing. Ability to work with individuals with intellectual and developmental disabilities, including those with seizure disorders and autism.
  • Physical Demands: Frequent travel, constant movement, and occasional lifting and positioning of patients.
