Job Overview

Essential Staff Qualifications

· Minimum age requirement of 18 years

· Verification of identity, current licensure, high school diploma or equivalent, and relevant degree is mandatory

· Successful completion of a criminal background investigation, EMR (Employee Misconduct Registry), and NAR (Nurse Aide Registry) is required

· Possession of a valid CPR and first aid certification is essential

· A TB test must be conducted within two weeks of employment and annually thereafter

Registered Nurse/LVN Qualifications (Full-Time or Part-Time)

  • Must hold a valid license from the Texas Board of Nursing and adhere to the Nurse Practice Act along with the regulations set forth by the Texas Board of Nursing

· High school diploma or equivalent is required

  • Current CPR certification and a negative TB test are necessary

· A minimum of two years of full-time experience in nursing, particularly with individuals with disabilities in a healthcare or human services setting

  • Availability for an 8-hour shift from Monday to Friday (7am – 5pm) is expected
  • If an LVN is appointed as the facility nurse, an RN consultant must provide oversight at the facility for at least 4 hours weekly, with documentation of the consultation provided when individuals are present

Nurse/LVN Responsibilities

· Conduct an initial assessment within 14 days of employment to identify the required care

· Secure physician's orders for medications and treatments to be administered

· Formulate an individualized care plan based on physician directives and participant needs

· Evaluate the feasibility of self-administered medications and ensure compliance

· Maintain documentation of monthly progress regarding medical care provided

· Provide training and guidance to the LVN to ensure accurate medical record-keeping

· Deliver health education to participants and direct service staff

· Keep comprehensive medical records

· Observe and document participant behavior, condition, and care provided on a daily basis

· Monitor vital signs, weight, blood pressure, and medications monthly for care plan review

· Offer counseling and education to participants and their families regarding their care plans

· Suggest tools or devices that may enhance the quality of life for participants

· Gather patient health histories and make informed treatment decisions when necessary

· Assist with outings and field trips

· Responsibilities may evolve as required

· Participate in 3 hours of ongoing training quarterly

· Engage in monthly evacuation procedure drills

· Complete fire, disaster, and evacuation training within three working days

· Accumulate 18 hours of training within the first three months

· Uphold confidentiality at all times
