We are seeking a skilled and dedicated Ambulatory Licensed Practical Nurse (LPN) to join our team at Community Health Network. In this role, you will work closely with physicians to deliver high-quality patient care and achieve exceptional outcomes.
Job SummaryThe Ambulatory LPN will assist healthcare providers with patient visits and procedures, as well as facilitate routine laboratory tests and referrals. This position requires excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

- Facilitate routine laboratory tests and referrals
- Provide exceptional patient care and support
To be successful in this role, you must have a strong foundation in nursing principles and practices. We require:
- A graduate degree from an accredited school of practical nursing
- Licensure as a Licensed Practical Nurse (LPN) by the Indiana Professional Licensing Agency (IPLA)
- Previous experience in a medical office or healthcare setting (preferred)
We offer a competitive salary package, including:
- $63,000 - $83,000 per year (estimated based on location and industry standards)
