Job DescriptionWe're seeking a skilled Healthcare Navigator - Clinical Operations to join our team. In this role, you'll communicate with patients telephonically, identify health gaps, and coordinate patient care. You'll work closely with clinical teams to deliver personalized care plans, ensuring seamless transitions between care settings.
Your responsibilities will include:
- Coordinating patient flow and scheduling appointments
- Interacting with patients via phone, email, and video conferencing
- Scheduling tests and reconciling referrals
- Demonstrating proficiency in Microsoft Office and electronic health records
You'll also adhere to care management processes, maintain confidentiality, and prioritize tasks effectively.
Requirements and QualificationsTo succeed in this role, you'll need:
- A high school diploma or equivalent (associate's degree preferred)
- 3-5 years of experience in healthcare, customer service, or patient relations
- Natural passion for engaging patients and improving health outcomes
- Strong communication, problem-solving, and organizational skills
Certification as a Medical Assistant or other relevant certifications are a plus.
