Job Summary:We are looking for a skilled Nurse Practitioner to implement palliative care program requirements, conduct face-to-face encounters with patients, and coordinate care with the Palliative Care team. The ideal candidate will have a Master's degree in Nursing, current licensure, and certification as an Adult or Family Nurse Practitioner.
Key Responsibilities:- Implement palliative care program requirements
- Conduct face-to-face encounters with patients to provide re-certification information to the Medical Director/Associate Medical Director
- Communicate with patients/families and all agency personnel
- Provide clinical findings to the Primary care physician and/or supervising Medical Director/Associate Medical Director
- Master's degree in Nursing required, with current licensure in the state of practice
- Certification as an Adult or Family Nurse Practitioner
- Experience in Hospice or Palliative Care nursing preferred
- Knowledge of the framework, function, and organization of a hospice care program
