Job SummaryWe are looking for a compassionate and experienced Registered Nurse to provide advanced illness management and support to patients and their families. As an Afterhours Palliative Care RN, you will be responsible for identifying and coordinating patient/family needs, providing preventive and supportive care, and utilizing assessment, intervention, and teaching skills to maximize patient comfort and enhance quality of life.
Responsibilities- Triage patient needs over the phone and make initial nursing evaluations to determine eligibility for palliative services
- Respond to crisis calls from patients and families after hours and provide timely documentation of patient/family response to palliative care
- Collaborate with patients, families, attending physicians, and other members of the interdisciplinary team to provide patient and family care
- Maintain accurate and relevant clinical notes regarding patient condition and communicate effectively with members of the AIM team, clinical supervisor, and patient's PCP
- Report patient condition and identified needs/issues to supervisor, nurse practitioner, or PCP
- Associates/Bachelor's degree in Nursing
- 2 years of prior skilled Registered Nurse experience
- Demonstrated ability to provide quality patient care
- Nursing experience in palliative care, oncology, hospice, or geriatric care preferred
