We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Gentiva, a leading provider of hospice and palliative care services. As a Nurse Practitioner - Advanced Illness Management Specialist, you will play a critical role in delivering high-quality, patient-centered care to individuals with advanced illnesses.
Key Responsibilities- Collaborate with our interdisciplinary team to develop and implement comprehensive care plans for patients with advanced illnesses.
- Provide advanced illness management and palliative care services, including diagnosis, treatment, and follow-up care.
- Work closely with patients, families, and caregivers to ensure that their needs are met and that they receive the highest level of care possible.
- Participate in ongoing education and training to stay up-to-date on the latest advances in palliative care and advanced illness management.
- Contribute to the development and implementation of quality improvement initiatives to enhance patient outcomes and satisfaction.
- Master's degree in Nursing from an accredited institution.
- Current licensure as a registered nurse and Nurse Practitioner in the state of practice.
- Minimum of two years of experience as a Nurse Practitioner in a palliative care or advanced illness management setting.
- Strong leadership and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Excellent communication and documentation skills, with the ability to maintain accurate and timely records.
- Knowledge of palliative care principles, including pain and symptom management, and the ability to apply this knowledge in practice.
