Job Summary

Northwell Health is seeking a skilled Clinical Social Worker - LMSW to join our team. As a key member of our healthcare team, you will provide case and group work services to patients and their families during their hospital stay. Your expertise will ensure continuity of medical care by referring patients to appropriate follow-up services.

Key Responsibilities

* Provide direct service to patients and their families using various social work modalities to help them maximize their functioning.
* Travel to patient homes, community resources, and community meetings may be required.
* Advocate on behalf of patients to ensure maximum access to healthcare.
* Maintain accurate and professional patient care documentation as defined by hospital policies and procedures.
* Function in a consultative and educative capacity with all relevant individuals or agencies impacting patients.
* Prepare materials for supervisory conferences.
* Direct the activities of paraprofessional staff, social work interns, and volunteers as needed.
* Participate in research, staff development programs, and community liaison activities as required.

Requirements

* Master's Degree in Social Work, required.
* LMSW required within one year of employment, required.
