Job Title: Licensed Clinical Social Worker

Sun River Health is seeking a part-time Licensed Clinical Social Worker to support our Spring Valley and Haverstraw, NY locations. The successful candidate will provide crisis intervention, bio psychosocial assessments, individual, family, and group counseling, and/or case management services to patients with issues related to chronic illness, aging, maternity, domestic violence, or other social problems.

Key Responsibilities:
  • Provides mental health services to patients with behavioral health conditions.
  • Develops and formulates treatment plans for patients in care, working with psychiatrists on a monthly basis.
  • Works with a multi-disciplinary team, including primary care providers.
  • Supports transition and coordination for chronically and persistently mentally ill patients to appropriate, alternate care.
  • Documents all patient encounters appropriately and maintains comprehensive case notes.
  • Reviews quality reports for site on all indicators monthly.
  • Completes monthly program reports.
  • Participates in regional network meetings.
  • Serves as site liaison with HRHCare/Genesis program staff.
  • Works cooperatively with various health population medical and Behavioral Health teams.
  • Participates in and facilitates case conferences, as well as other agency, school, and/or community-based meetings or trainings.
Requirements:
  • Master's Degree (MSW) in Social Work.
  • Licensure/Certification/Registration: Must be a LCSW in the State of New York, CPR/BLS Certification.
  • Minimum Work Related Experience: 2-4 years.
  • Preferred Work Related Experience: 5-8 years.
  • Specialized Skills and Knowledge: Bi-lingual, Spanish speaking is preferred but not required.
  • Ability to travel to both Spring Valley, NY and Haverstraw, NY.
Salary Range:

$43 - $52 per hour
