Job Summary

We are seeking a skilled Social Worker to join our team at Northwell Health. As a Clinical Social Worker, you will provide case and group work services to patients and their families during their hospital stay.

Key Responsibilities:

  • Provide direct service to patients and their families using various social work modalities to help clients maximize their functioning.
  • Travel to patient homes, community resources, and community meetings as needed.
  • Advocate on behalf of the client to ensure maximum access to healthcare.
  • Maintain appropriate professional patient care documentation as defined by hospital policies and procedures.
  • Function in a consultative and educative capacity with all pertinent individuals or agencies that impact on patients.
  • Prepare materials for supervisory conferences.
  • May direct the activities of paraprofessional staff, social work interns, and volunteers.
  • May participate in appropriate research, staff development programs, and community liaison activities.

Requirements:

  • Master's Degree in Social Work, required.
  • LMSW required within one (1) year of employment, required.

Salary Information:

The salary range for this position is $38.28-$52.51/hour.
