Job Summary

We're looking for a seasoned Case Management professional to lead our team in delivering high-quality patient care. As a Manager, Case Management, you will be responsible for overseeing the daily operations of our Case Management department, ensuring that our patients receive the best possible care. You will work closely with our healthcare teams to develop and implement effective care plans, manage patient flow, and optimize resource utilization.

Key Responsibilities
  • Lead and manage a team of Case Managers and Clinical Social Workers to ensure seamless patient care.
  • Develop and implement operational standards and processes to ensure compliance with regulatory guidelines.
  • Collaborate with healthcare teams to develop and implement effective care plans.
  • Manage patient flow and optimize resource utilization to ensure efficient use of resources.
  • Monitor and analyze data to identify trends and areas for improvement.
  • Develop and implement strategies to improve patient outcomes and satisfaction.
  • Collaborate with leadership to develop and implement departmental policies and procedures.
Requirements
  • 3-5 years of relevant management and Case Management experience.
  • Current or compact RN licensure in the state of North Carolina or current licensure as a licensed clinical social worker by the NC Social Work Certification and Licensure Board.
  • Strong leadership and communication skills.
  • Ability to analyze data and develop effective solutions.
  • Knowledge of healthcare regulations and accreditation standards.
