Holistic Care Coordinator

Are you a passionate mental health professional seeking a fulfilling career where you can make a difference? We are currently seeking a Holistic Care Coordinator to join our dynamic team as a Qualified Mental Health Professional-Community Services (QMHP-CS).
Annual Salary: $58,000-$65,000
About the RoleThe successful candidate will have a Master's degree in a relevant field or a bachelor's degree in an approved human services field with relevant experience. They will be responsible for delivering holistic care that addresses the mental health, educational, and social needs of individuals within the community.
Candidates will work collaboratively with the clinical team, including psychiatrists, to deliver comprehensive care. Strong clinical skills and a commitment to client-centered care are essential for this role.
Main Responsibilities- Develop and implement individualized treatment plans for clients
- Utilize evidence-based practices to provide high-quality mental health services
- Collaborate with the clinical team, including psychiatrists, to deliver comprehensive care
- Maintain accurate and timely documentation
- Work closely with clients to achieve their goals and promote recovery
- Master's degree in a relevant field preferred, or a bachelor's degree in an approved human services field with relevant experience
- Valid QMHP-CS credential
- Demonstrated competency in providing community-based mental health services
- Strong clinical skills and a commitment to client-centered care
- Excellent communication, collaboration, and documentation abilities
