Job Description:

We are looking for a highly skilled Nurse Care Coordinator who can manage member cases, provide supervision and direction to non-RN clinicians, develop and monitor care plans, and perform face-to-face clinical assessments.

Key Responsibilities:

Identify members at high risk of complications and coordinate care with the healthcare teamManage members with chronic illnesses, co-morbidities, and disabilities to optimize healthcare utilizationDevelop individualized care plans and ensure access to necessary servicesInterface with Medical Directors, Physician Advisors, and Inter-Disciplinary Teams on care management treatment plans

Requirements:

  • RN license in applicable state(s) required
  • Minimum 3 years of experience in Service Coordinator, Case Management, or similar role
  • MA/MS in Health/Nursing preferred

Estimated Salary: $85,000 - $110,000 per year
