Nurse Specialist in Diabetes Care

The RN-Diabetes Care & Education Specialist plays a vital role in delivering comprehensive and seamless services that bridge the gap between clinical and self-management aspects of diabetes and cardiometabolic care. This position is an integral part of the interprofessional team, providing collaborative, comprehensive, and person-centered care and education to promote behavior change and improved quality of life across the lifespan.
Key Responsibilities- Assess the physical, cognitive, emotional, and educational needs of the diabetic population served.
- Develop individualized plans of care in collaboration with physicians and other healthcare professionals.
- Develop, evaluate, and revise diabetes education programs in accordance with ADA standards, JCAHO standards, and Medicare guidelines.
- Utilize clinical judgment and critical thinking skills to assess and analyze patient data, guiding patients in making personal goals to improve their health and better manage their diabetes.
- Participate in process improvement projects, using data to improve care processes and report results to the Diabetes Advisory Committee.
- Associate's Degree in Nursing.
- 3 years of experience in Nursing, including experience in diabetes education.
- BSN.
This is a non-management position that reports to a supervisor, manager, director, or executive.
