We are seeking a skilled Registered Nurse to join our team as a Diabetes Care Specialist. In this role, you will play a critical part in delivering comprehensive and seamless services that bridge the gap between clinical and self-management aspects of diabetes and cardiometabolic care.
Key Responsibilities:
* Collaborate with the interprofessional team to provide person-centered care and education conducive to behavior change and improved quality of life
* Assess the physical, cognitive, emotional, and educational needs of the diabetic population
* Develop and implement competency-based education practices and standards of care for GHS physician practices and departments caring for diabetic patients
* Utilize clinical judgment and critical thinking skills to assess and analyze patient data and evaluate patient responses to therapy
Requirements:
* Associate's Degree in Nursing
* 3 years of experience in Nursing, including experience in diabetes education
* Certified Diabetes Educator
* Current South Carolina RN License
