Job Summary

The Diabetes Care and Education Specialist plays a critical role in delivering comprehensive and seamless services that bridge the gap between clinical and self-management aspects of diabetes and cardiometabolic care in various settings.

The ideal candidate will be an integral part of the interprofessional team, providing collaborative, comprehensive, and person-centered care and education that fosters behavior change and improves quality of life across the lifespan.

Key Responsibilities
  • Assess the physical, cognitive, emotional, and educational needs of the diabetic population served by the program.
  • Develop and implement individualized plans of care in collaboration with physicians and other healthcare professionals.
  • Develop, evaluate, and revise diabetes education programs in accordance with ADA standards, JCAHO standards, and Medicare guidelines.
  • Utilize clinical judgment and critical thinking skills to assess and analyze patient data and evaluate patient responses to therapy.
  • Guide patients in making personal goals to improve their health and better manage their diabetes.
  • Participate in process improvement projects and utilize data to improve care processes.
Requirements
  • Associate's Degree in Nursing
  • 3 years of experience in Nursing, including experience in diabetes education
  • Certified Diabetes Educator (CDE) certification
  • Current South Carolina RN License
