Job Summary

Lexington Medical Center is seeking a highly skilled Diabetes Care Specialist to join our team. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and education to individuals with diabetes. Your expertise will help patients achieve optimal health outcomes and improve their quality of life.

Key Responsibilities
  • Develop and implement individualized diabetes care plans for patients
  • Provide education and support to patients and their families on diabetes management and self-care
  • Collaborate with healthcare providers to ensure comprehensive care and coordination
  • Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as needed
  • Stay up-to-date with the latest research and best practices in diabetes care and education
Requirements
  • Bachelor's degree in Dietetics or a related field
  • 2+ years of experience as a Certified Diabetes Care and Education Specialist or Clinical Dietitian
  • Certification as a Diabetes Care and Education Specialist (or obtain within 6 months of hire)
  • CPR certification
