Geriatric Medicine Position in Outpatient Care

Position Overview:
At Sage Health, our dedicated care team is composed of Physicians, Advanced Practice Professionals, Nurses, Social Workers, Case Managers, Care Coordinators, Medical Assistants, Behavioral Health Support, quality-focused Coders, Referral Coordinators, and more. This extensive support system enables our primary care physicians to manage a smaller patient load, allowing for more meaningful patient interactions.
Key Responsibilities:
- Assess and treat patients in alignment with established care standards.
- Facilitate the coordination of patient services, including specialty referrals, hospital and skilled nursing facility coordination, durable medical equipment, and home health services.
- Actively engage as a key medical resource within the care team during daily discussions.
- Support the Lead Physician and Center Administrator in fostering a collaborative practice environment through professionalism and approachability.
- Ensure timely completion of all medical documentation, collaborating with a quality-focused coder to enhance thorough documentation.
- Adhere to policies and protocols as outlined by Clinical Leadership.
- Regularly meet with the Lead Physician to discuss quality of care, review outcome metrics, and address medical record concerns.
- Maintain a clinical focus on direct patient care, balancing patient-facing activities with necessary administrative tasks (such as charting and meetings).
Required Qualifications:
- Graduation from an accredited MD or DO program.
- Possession of a current and unrestricted medical license or willingness to obtain one in the state of practice; readiness to secure licenses in additional states as needed.
- Board Certification or eligibility for certification (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine is preferred.
- Active and unrestricted DEA license.
- Medicare and Medicaid Provider Numbers.
- Exceptional interpersonal skills and communication abilities with colleagues and patients.
- Commitment to a team-based care model.
- Willingness to learn and adapt to a value-based care environment.
- Outstanding patient and customer service skills.
- Basic computer proficiency, including familiarity with email and electronic medical records.
Preferred Qualifications:
- Familiarity with HEDIS/STARS metrics.
- Understanding of CAHPS and HOS.
- Experience in medically underserved or culturally diverse communities.
- Experience with electronic health records.
- Experience in Medicare Advantage Global Risk Models.
