Outpatient Geriatric Medicine Physician Opportunity in Pinebrook/Mobile, AL

Sage Health is seeking a skilled Geriatric Medicine Physician to join our team in Pinebrook/Mobile, AL. As a key member of our care team, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.
Key Responsibilities- Evaluate and Treat Patients: Assess and treat patients in accordance with established standards of care, ensuring the highest level of patient satisfaction and outcomes.
- Coordinate Patient Services: Assist in coordinating patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care, to ensure seamless care delivery.
- Collaborate with the Care Team: Act as an active participant and key source of medical expertise with the care team through daily huddles, ensuring effective communication and coordination of patient care.
- Lead by Example: Help set a tone of cooperation in practice by displaying a professional and approachable demeanor, promoting a positive work environment.
- Document Patient Care: Complete all medical record documentation daily, working with a quality-based coder to optimize comprehensive documentation and ensure accurate billing.
- Follow Policy and Protocol: Adhere to policy and protocol defined by Clinical Leadership, ensuring compliance with regulatory requirements and organizational standards.
- Meet with Lead Physician: Meet with the Lead Physician to discuss quality of care, review outcome data, and address medical records issues, ensuring continuous improvement and quality patient care.
- Focus on Direct Patient Care: Prioritize direct patient care, inclusive of patient-facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care.
- Medical Degree: Graduate of an accredited MD or DO program of an accredited university.
- Medical License: Current and unrestricted medical license or willingness to obtain a medical license in the state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required.
- Board Certification: Board Certification or Eligibility to become certified (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine preferred.
- DEA License: Active and unrestricted DEA license.
- Medicare and Medicaid Provider Numbers: Medicare Provider Number and Medicaid Provider Number.
- Interpersonal Skills: Demonstrate a high level of skill with interpersonal relationships and communications with colleagues/patients.
- Team-Based Care: Fully engaged in the concept of team-based care model.
- Adaptability: Willingness and ability to learn/adapt to practice in a value-based care setting.
- Superior Patient Service: Superior patient/customer service.
- Basic Computer Skills: Basic computer skills, including email and EMR.
- HEDIS/STARS Knowledge: Knowledge of HEDIS/STARS.
- CAHPS and HOS Knowledge: Knowledge of CAHPS and HOS.
- Medically Underserved Communities Experience: Experience working in medically underserved/culturally diverse communities.
- Electronic Health Record Experience: Experience working with an electronic health record.
- Medicare Advantage Global Risk Models Experience: Experience working in Medicare Advantage Global Risk Models.
