Outpatient Geriatric Medicine Physician Opportunity in Pinebrook/Mobile, AL

About the Role:
Sage Health is seeking a skilled Geriatric Medicine Physician to join our team in Pinebrook/Mobile, AL. As a key member of our care team, you will play a vital role in delivering high-quality patient care and contributing to our mission of providing exceptional healthcare services.
Key Responsibilities:
- Evaluate and Treat Patients: You will be responsible for evaluating and treating patients in accordance with established standards of care, ensuring that each patient receives the highest level of care and attention.
- Coordinate Patient Services: You will assist in coordinating patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care, to ensure seamless care delivery.
- Collaborate with the Care Team: As an active participant and key source of medical expertise, you will work closely with the care team to ensure that patients receive comprehensive and coordinated care.
- Lead by Example: You will help set a tone of cooperation and professionalism within the practice by displaying a positive and approachable demeanor.
- Document Patient Records: You will complete all medical record documentation daily, working with a quality-based coder to optimize comprehensive documentation and ensure accurate billing.
- Follow Policy and Protocol: You will adhere to policy and protocol defined by Clinical Leadership, ensuring that all care is delivered in accordance with established standards.
- Meet with Lead Physician: You will meet with the Lead Physician to discuss quality of care, review outcome data, and address medical records issues.
- Clinical Focus: You will maintain a clinical focus on direct patient care, including patient-facing time and general administrative time related to direct patient care.
Required Qualifications:
- Medical Degree: You must be a graduate of an accredited MD or DO program from an accredited university.
- Medical License: You must hold a current and unrestricted medical license or be willing to obtain a medical license in the state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required.
- Board Certification: Board Certification or Eligibility to become certified (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine is preferred.
- DEA License: You must hold an active and unrestricted DEA license.
- Medicare and Medicaid Provider Numbers: You must have a Medicare Provider Number and a Medicaid Provider Number.
- Interpersonal Skills: You must demonstrate a high level of skill with interpersonal relationships and communications with colleagues and patients.
- Team-Based Care: You must be fully engaged in the concept of team-based care model.
- Adaptability: You must be willing and able to learn and adapt to practice in a value-based care setting.
- Customer Service: You must demonstrate superior patient/customer service skills.
- Computer Skills: You must have basic computer skills, including email and EMR.
Preferred Qualifications:
- HEDIS/STARS Knowledge: Knowledge of HEDIS/STARS is preferred.
- CAHPS and HOS Knowledge: Knowledge of CAHPS and HOS is preferred.
- Medically Underserved Communities Experience: Experience working in medically underserved/culturally diverse communities is preferred.
- Electronic Health Record Experience: Experience working with an electronic health record is preferred.
- Medicare Advantage Global Risk Models Experience: Experience working in Medicare Advantage Global Risk Models is preferred.
