Sage Health is seeking a skilled Geriatric Medicine Physician to join our team in Mobile, AL. As a key member of our care team, you will play a vital role in providing high-quality patient care and contributing to our mission of delivering exceptional healthcare services.
Responsibilities:- Evaluate and treat patients in accordance with standards of care, ensuring the highest level of patient satisfaction and outcomes.
- Coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care, to ensure seamless care transitions.
- Participate in daily huddles with the care team to share medical expertise and collaborate on patient care plans.
- Work closely with the Lead Physician and Center Administrator to establish a positive and cooperative team environment.
- Document patient records accurately and efficiently, working with a quality-based coder to optimize comprehensive documentation.
- Adhere to policy and protocol defined by Clinical Leadership, ensuring compliance with regulatory requirements.
- Meet with the Lead Physician to discuss quality of care, review outcome data, and address medical records issues.
- Graduate of an accredited MD or DO program from a reputable university.
- Current and unrestricted medical license, with eligibility to obtain licenses in other states as required.
- Board Certification or Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine, with a strong preference for ABMS or AOA certification.
- Active and unrestricted DEA license, as well as Medicare and Medicaid Provider Numbers.
- Excellent interpersonal and communication skills, with a strong ability to work effectively with colleagues and patients.
- Familiarity with team-based care models and a willingness to adapt to a value-based care setting.
- Superior patient/customer service skills, with a focus on delivering exceptional care and satisfaction.
- Basic computer skills, including email and EMR, with a strong ability to learn and adapt to new systems.
- Knowledge of HEDIS/STARS and CAHPS/HOS, with experience working in medically underserved and culturally diverse communities.
- Experience working with electronic health records and Medicare Advantage Global Risk Models.
