Personal Care Assistant

Schedule: Monday-Friday 7AM-11AM including weekend/holiday rotations
Incentive: $500 Sign-On Bonus
A Personal Care Assistant plays a vital role in providing in-home support to participants of Neighborhood PACE, assisting with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). This position requires the ability to work autonomously and efficiently in a minimally supervised environment, demonstrating effective problem-solving skills while knowing when to seek guidance from supervisory personnel. Respect and appreciation for the participant's home and community are essential.
Key Responsibilities:
- Regularly reviews and monitors participant care plans, ensuring adherence to the established plan of care. Accurately documents the care provided on participant flow sheets.
- Checks the health aide voicemail at the start of each shift and every two hours thereafter. Timely reports any unusual findings, changes in participant status, or care refusals to the team via the appropriate communication channels.
- Assists participants with ADLs as outlined in the care plan, including requests for help with toileting and other personal needs.
- Accompanies participants to medical appointments as required.
- Performs basic treatments under the supervision of nursing staff, such as simple catheter care and skin assessments during personal care.
Qualifications:
- Current certification as a Home Health Aide (H.H.A.) or Certified Nursing Assistant (C.N.A.) from an accredited program.
- Demonstrated competencies in providing participant care.
- A minimum of one year of experience working with elderly or frail populations.
- Previous experience in a health aide, personal care assistant, or similar role.
- Must possess a vehicle for travel between participant homes.
- Strong customer service and communication skills; bilingual abilities are a plus.
- Physical capability to assist with ADLs and mobility for all PACE participants.
