Job Title: Clinical Care Manager - Long Term Care
Job Summary: We are seeking a skilled Clinical Care Manager to join our team. As a Clinical Care Manager, you will be responsible for coordinating care for our members, working closely with healthcare providers, and ensuring that our members receive the best possible care.
Key Responsibilities:
- Conduct thorough assessments of members' needs and develop personalized care plans
- Collaborate with healthcare providers to ensure seamless care coordination
- Monitor members' progress and make adjustments to care plans as needed
- Authorize and coordinate referrals for services
- Ensure that members receive high-quality, patient-centered care
Requirements:
- 3+ years of care management experience
- Home health, discharge planning, or long term care experience preferred
- NYS RN license required
