Community Support Specialist

CAMBA is an innovative organization dedicated to improving the health outcomes for individuals with severe mental illness & other complex chronic illnesses through coordinated networks of medical, psychiatric, behavioral health, social service & housing providers.
We are looking for a skilled Community Support Specialist to join our Health Home program, which places us at the forefront of a broad system-wide change that will have a significant impact on many human services sectors.
The ideal candidate will have an Associate's Degree or equivalent experience in a related field, as well as knowledge of HIV/AIDS and related issues. Bilingual English speakers with proficiency in Spanish, Russian, Haitian-Creole, or Chinese (Mandarin or Cantonese) are preferred.
This is a full-time position with an estimated salary of $51,000 annually, plus benefits including health insurance, dental insurance, and a 403(b) retirement plan with employer match.
Responsibilities- Carry a caseload of approximately 25-35 clients.
- Act as client liaison/client advocate with outside organizations regarding such matters as education, healthcare, housing, legal issues, entitlements, etc.
- Follow-up with clients for a period of time after successful completion of their primary goals to assure client stability.
