  • Schedule appointments every 90 days or as required
  • Organize transportation for members when needed
  • Identify gaps in care or health requirements
  • Utilize resources to assist with specific health care needs and support caregivers in managing daily challenges
  • Coordinate transitions between care settings, including hospital discharges
  • Evaluate eligibility for long-term care services and support
  • Connect with community resources
  • Locate services from additional resources, including community and social service programs such as physical therapy or meal delivery services
  • Arrange services with primary care providers, family members, caregivers, and other identified providers

BA in relevant fields such as social work or health administration.
