Care Transition Specialist Lead

Brigham and Women's Hospital is seeking a highly skilled Care Transition Specialist Lead to join our team. As a key member of our Care Continuum Management team, you will play a critical role in ensuring seamless transitions for patients from our hospital to community-based care settings.

Key Responsibilities:
  • Coordinate patient transfers to extended care facilities, home healthcare agencies, and other community-based care settings.
  • Develop and implement discharge plans in collaboration with interdisciplinary teams.
  • Act as a liaison between hospital staff, patients, families, and community-based care providers.
  • Monitor and evaluate the quality of care provided in extended care facilities and home healthcare agencies.
  • Participate in performance improvement initiatives to optimize patient outcomes and reduce readmissions.
Requirements:
  • Bachelor's degree in a healthcare-related field.
  • 3-5 years of experience in hospital discharge planning, extended care facilities, or community health.
  • Excellent interpersonal and communication skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Basic typing and computer skills.
