Nurse Team Lead

Mass General Brigham is seeking a skilled Registered Nurse (RN) to lead our Progressive Care Unit team. As a seasoned nurse with a passion for patient care, you will be responsible for coordinating patient transitions, managing referrals, and ensuring seamless care delivery.
Key Responsibilities:
- Coordinate patient transitions across Mass General Brigham, ensuring timely and effective care delivery.
- Manage referrals to extended care facilities, homecare agencies, and other community resources.
- Develop and maintain relationships with social agencies, clinics, schools, and courts to ensure comprehensive patient care.
- Monitor quality of care in extended care facilities and homecare agencies, reporting findings to the Program Manager.
- Provide training and support to Care Transition Specialists and other department roles.
Requirements:
- Bachelor's Degree in Nursing or related field.
- 3-yr experience in hospital discharge planning, long term care facility, community health, or utilization review.
- Excellent interpersonal skills, with ability to interact effectively with various levels of staff, patients, families, and community organizations.
- Extensive knowledge of regulations, community organizations, state and federal systems, medical terminology, and levels of healthcare.
