We are seeking a highly skilled Cardiology Clinic Nurse to join our team at Tufts Medical Center. As a key member of our care team, you will be responsible for coordinating the transition of care from one healthcare setting to another, ensuring seamless transitions for our patients.
Key Responsibilities- Risk Assessment and Management: Perform risk assessments using standardized tools to identify patients at high risk for readmission or complications.
- Transition of Care: Collaborate with the care team to develop discharge care plans, coordinate post-discharge care, and ensure medication reconciliation.
- Post-Discharge Care: Call patients at pre-determined intervals to ensure adherence to care plans, resolve obstacles to adherence, and promote patient wellbeing.
- Data Collection and Analysis: Collect and document key data points into appropriate systems, participate in quality assurance activities, and recommend improvements to care processes.
- Bachelor of Science degree in Nursing
- Minimum of 5 years of clinical nursing experience working with heart failure cardiology patient populations
