Job Summary

We are seeking a highly skilled and experienced Care Transition Specialist, Lead to join our team at Mass General Brigham. As a key member of our Care Continuum Management team, you will play a critical role in ensuring seamless transitions for patients from inpatient to community settings.

Key Responsibilities
  • Manage system referrals, escalations, and support patient progression
  • Act as an advocate for patients and patient families, promoting optimal resource management, high customer satisfaction, and high-quality care
  • Collaborate with Case Managers, Social Workers, and other care team staff to ensure patients receive necessary resources and services
  • Analyze operational data to evaluate performance and identify areas for improvement
  • Develop and maintain relationships with community organizations, state regulatory agencies, and non-acute care provider agencies
Requirements
  • Bachelor's Degree in a related field and 3-5 years of experience in hospital discharge planning, long-term care facility, community health, or utilization review
  • Extensive knowledge of regulations, community organizations, state and federal systems, medical terminology, and levels of healthcare
  • Strong interpersonal and communication skills, with ability to work effectively in an interdisciplinary team setting
  • Basic typing and computer data entry skills, with experience with personal computers and software desirable
