Key Responsibilities- Conduct Home Visits and Assessments: Conduct thorough home visits and assessments to determine the safety and appropriateness of child placements, connecting children to community services to address risk factors.
- Ongoing Case Management: Provide ongoing case management with a focus on screening and making referrals to address child needs in areas such as placement issues, safety concerns, education, health, mental health, legal proceedings, and more.
- Crisis Intervention: Provide crisis intervention services directly or connect families to services to ensure minor safety and well-being in crisis situations.
- Documentation and Reporting: Submit detailed and thorough documentation in compliance with grantor policies and procedures.
- Collaboration and Communication: Maintain regular and timely communication with supervisors regarding child safety, stability, or well-being in placements, and participate in ongoing supervision.
- Education: Bachelor's degree in Social Work or other relevant degree.
- Language Skills: Bilingual in Spanish/English.
- Driver's License and Vehicle: Valid driver's license and reliable vehicle.
- Certifications and Background Checks: Must be able to pass an FBI background check and a Child Abuse/Neglect check for every state in which they have resided in the last five years.
- Experience: At least one year of experience with child welfare programs or as a Case Manager.
