Hospice Care Professional

Covenant Hospice, a renowned healthcare provider, seeks an exceptional professional to deliver compassionate care and support to patients nearing the end of life. This is a part-time opportunity that requires a dedicated individual with a strong commitment to patient-centered care.
Estimated Salary: $35.00 per hour, based on industry standards in Phoenix, AZ 85051.
Responsibilities- Work collaboratively with the Interdisciplinary Team to provide comprehensive care, ensuring the physical, emotional, and spiritual needs of patients are met.
- Assess patients for appropriateness of hospice care, utilizing LCDs, federal regulations, and clinical knowledge as guidelines.
- Develop and implement personalized plans of care, addressing pain management, symptom control, and quality of life enhancement.
- Provide education and support to patients and their families, promoting understanding of the hospice philosophy and its benefits.
- Participate in Interdisciplinary Team meetings, contributing to the development of care plans and ensuring seamless coordination of services.
- Maintain accurate records, communicating effectively with patients, families, and healthcare professionals.
- Registered Nurse (RN) from an accredited program, licensed and in good standing with the State Board of Nursing.
- At least 1 year of experience in acute care or home health/hospice setting, with a demonstrated ability to work cooperatively with interdisciplinary teams.
- BLS Certification and RN licensure preferred.
