About the Role

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Nurse Specialist. In this role, you will have the opportunity to make a meaningful difference in the lives of patients and their families.

Job Summary

The successful candidate will provide high-quality patient care, working closely with our interdisciplinary team to develop and implement individualized plans of care. You will be responsible for assessing patients' physical, emotional, and spiritual needs, and providing education and support to patients and their families.

Key Responsibilities
  • Conduct comprehensive assessments of patients' needs, identifying areas for improvement and developing effective care plans
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians
  • Collaborate with patients, families, and healthcare professionals to ensure seamless care coordination and communication
  • Maintain accurate and up-to-date records of patient progress, including medical history, treatment plans, and outcomes
Requirements
  • Current Registered Nurse (RN) license for the state in which the nurse practices
  • At least one year of experience in hospice or palliative care
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
  • Strong critical thinking and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities
