Job Summary

We are seeking a highly skilled Registered Nurse (RN) to serve as a Care Coordinator for our hospice services. This role requires a compassionate and detail-oriented individual who can effectively manage patient care plans, coordinate with interdisciplinary teams, and provide emotional support to patients and their loved ones.

Responsibilities
  • Clinical Assessment and Planning: Conduct thorough assessments of patients' medical needs, develop personalized care plans, and ensure timely implementation of treatments and interventions.
  • Collaboration and Communication: Work closely with Clinical Directors, nurses, and other healthcare professionals to ensure seamless coordination of care and effective communication with patients and families.
  • Patient Support and Education: Provide emotional support, education, and guidance to patients and their families, empowering them to make informed decisions about their care.
  • Nursing Skills and Expertise: Utilize advanced nursing skills and knowledge to deliver high-quality care, address complex medical issues, and promote patient comfort and well-being.
  • Administrative Duties: Manage electronic medical records, maintain accurate documentation, and perform administrative tasks as needed.
Qualifications
  • Licensure: Current, unencumbered RN licensure in the state where services are provided.
  • Experience: Minimum one year of experience as an RN in a medical surgical or acute care setting; hospice experience preferred.
  • Computer Proficiency: Proficient in typing and various computer programs, including electronic medical records (EMR).
  • CPR Certification: Possess current CPR certification if required by state.
Salary and Benefits

We offer a competitive salary range of $60,000 - 80,000 per year, depending on experience, as well as a comprehensive benefits package, including health insurance, paid time off, and opportunities for professional growth and development.
