Salary Range:

$62,000 - $85,000 per year

About the Role

We are seeking a highly skilled Substance Use Specialist - Integrated Care Coordinator to join our interdisciplinary care team. As a key member of our team, you will play a vital role in providing individualized support to persons served, promoting their recovery, health, and overall wellbeing.

Responsibilities:
  • Engage persons served in therapeutic interactions to build trusting relationships, fostering a safe environment for them to explore their substance use practices and experiences.
  • Provide client-centered, harm reduction-oriented crisis intervention, individual counseling, and on-demand clinical services tailored to meet the unique needs of each person.
  • Support individuals in initiating and maintaining changes based on their individual goals, empowering them with the necessary information to make informed decisions about their health and wellbeing.
  • Lead group-level interventions with clients, creating a safe space for individuals to share and reflect with their peers on topics related to their substance use practices and experiences.
Requirements:
  • A strong foundation in substance use treatment, preferably with experience working in an outpatient clinic setting.
  • Excellent communication and interpersonal skills, enabling effective collaboration with diverse stakeholders, including clients, healthcare professionals, and community partners.
  • Ability to maintain confidentiality of records and persons served, adhering to strict standards of professionalism and respect.
  • Familiarity with integrated care principles, including coordinated service delivery, case management, and population health approaches.
