Dialysis Care Coordinator

Nurse Dialysis Specialist Job Description:
This role involves coordinating the total nursing care for End Stage Renal Disease (ESRD) patients on chronic dialysis by assessing, planning, implementing, and evaluating care in accordance with established hospital and departmental policies and procedures.
Main Responsibilities:
- Provides dialysis education to patients and families.
- Coordinates the work of the healthcare team by working cooperatively with other renal team personnel.
- Evaluates effectiveness of patient care and identifies opportunities for improvement.
Requirements and Qualifications:
- Licensure as a Registered Nurse in the State of Rhode Island or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement.
- Baccalaureate in Nursing preferred; Certified Nephrology Nursing Certification preferred.
- At least 12 months of nursing experience and an additional 3 months of specialized dialysis experience providing clinical nursing care to dialysis patients.
Estimated Salary: $65,000-$90,000 per year
