Avera is seeking a Community Health Worker who has lived life experience as an immigrant/refugee and who has navigated the challenges of life in a new setting. This individual will work closely with medical providers, primary care teams, and social service agencies to provide care coordination and connections to improve their health and general well-being through education and provision.

Key Responsibilities
  • Facilitate access to health and social services for underserved populations
  • Provide education and support to individuals and families to improve their health and well-being
  • Work collaboratively with healthcare teams to coordinate care and services
  • Develop and maintain relationships with community partners and stakeholders
Requirements
  • High School Diploma or Equivalent required
  • A minimum of two years' direct experience preferred
  • Experience with navigating local medical and social support systems preferred
  • Possible bilingual and medical interpreter qualifications preferred based on specific position needs
  • Possible knowledge and/or experience with certain medical diagnoses preferred based on specific position needs
  • Must be from the community that is being served or have a familiarity of the community

Work Schedule
  • Day Shift: Monday-Friday; 80 hours per 2 weeks

Pay

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
