Job Summary

The Clinical Pharmacist Specialist will play a critical role in our healthcare team, working closely with patients, physicians, and other healthcare professionals to provide optimal medication management and promote medication safety. This position requires a strong commitment to patient-centered care, excellent communication skills, and the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Provide optimal medication management and promotion of medication safety
  • Perform patient assessments and participate in direct patient care activities
  • Monitor therapy for appropriateness, dosage, effectiveness, interactions, and adverse drug reactions
  • Review and act upon clinical decision support surveillance
  • Complete organizational and/or departmental required documentation
  • Prescribe and/or administer medications as allowed by law and scope of practice
  • Maintain up-to-date, evidence-based knowledge required for services provided
Requirements
  • 4-Year/Bachelor's Degree in Pharmacy or Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification
  • 1 year of experience as a registered pharmacist in health system pharmacy
  • Current Pharmacist licensure in the state of Virginia
  • Board of Pharmacy Specialties Certification or other applicable certification
  • American Heart Association (AHA) Health Care Provider BLS certification
  • American Heart Association (AHA) Health Care Provider ACLS certification
Physical Demands

The job requires frequent sitting and standing, occasional walking, and bending/stooping. Frequent repetitive arm, hand, and finger movements. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull up to 20 lbs.
