Clinical Pharmacist

The Clinical Pharmacist will serve as a key member of the healthcare team, supporting personalized relationships with patients and their chosen support system. The pharmacist will collaborate with team members to assess, plan, implement, and evaluate an individualized plan of care that promotes optimal health.
The pharmacist will be responsible for managing and providing care in accordance with current policies and procedures. This will include providing optimal medication management and promoting medication safety, performing patient assessments, and participating in direct patient care activities and rounding opportunities.
Key Responsibilities- Provides optimal medication management and promotion of medication safety
- Performs patient assessments
- Participates in direct patient care activities and rounding opportunities
- Monitors therapy for appropriateness, dosage, effectiveness, interactions, and adverse drug reactions
- Reviews and acts upon clinical decision support surveillance
- Completes organizational and/or departmental required documentation
- Prescribes and/or administers medications as allowed by law and scope of practice
- Maintains up-to-date, evidence-based knowledge required for services provided
- 4-Year/Bachelor's Degree required - Bachelor's Degree in Pharmacy, Foreign Pharmacy Graduate Equivalency Committee (FPGEC) Certification
- 1 year of experience required - Postgraduate year 1 residency or fellowship OR a minimum of 2 years of experience as a registered pharmacist in health system pharmacy
- Current Pharmacist licensure in the state of Virginia completed within 30 days of start date by either examination or reciprocity
- Board of Pharmacy Specialties Certification or other applicable certification must be obtained and continuously maintained within 2 years of start date as applicable to the role required; excludes PRN team members who may not be eligible to take certification exam
