About the Role

Prairie Quest Consulting is seeking a highly skilled Transition Coordinator to join our team. As a Transition Coordinator, you will play a critical role in supporting students with disabilities in their transition to post-school outcomes.

Key Responsibilities
  • Student Support: Utilize a comprehensive screening assessment to gauge student workplace readiness and develop individualized employment plans to promote student success.
  • Case Management: Write detailed case notes and maintain accurate records of student progress, highlighting challenges and achievements.
  • Transition Planning: Assist students in identifying barriers and delivering tools and techniques to overcome them, promoting personal and professional development.
  • Resource Development: Help students identify resources within their family, community, and school system to prepare for employment, education, and training.
  • Financial Literacy: Provide training on money management skills to equip students with the knowledge and confidence to make informed financial decisions.
  • Transportation Support: Assist students in exploring local transportation options and provide instruction on using public transportation to access work and the community.
Requirements
  • Experience: Previous experience working with individuals with disabilities is essential.
  • Communication Skills: Strong oral and written communication skills are required to effectively interact with students, families, and community stakeholders.
  • Technology Proficiency: Proficiency in using various technology and tools is necessary to maintain accurate records and communicate with stakeholders.
  • Certifications: A valid driver's license and ability to pass a mandatory background screening are required.
